INSTRUCTIONS FOR NEW CUSTOMERS
If this is your first test order, you should read the "Instructions for New Customers" section of this webpage before placing your first order. After you have faxed or mailed Behavior Data Systems, Ltd. (BDS) a signed agreement, you become a "current customer" or a BDS test user. Test users can call (602) 234-3506, fax (602) 266-8227 or mail in their test orders.
NEW CLIENT TEST ORDER PROCEDURE
Since this is your first test order, you will need to fill out and mail or fax a signed BDS agreement before ordering tests. The following step-by-step instructions will guide you through the process of completing your first order.
In this webpage, a BDS agreement is presented for your review. Also included are the instructions for printing out a copy of this agreement for your signature and transmission to Behavior Data Systems.

Instructions: The following step-by-step instructions take you through the process of obtaining, filling out and sending your signed copy of the BDS agreement to Behavior Data Systems.
There are three ways to obtain the Behavior Data Systems, Ltd. agreement on this webpage:
1. Download a copy of the agreement (transmit a copy from this website to your computer) in Microsoft Word format.
2. Print a copy directly from this webpage.
3. Contact Behavior Data Systems by telephone (602) 234-3506, fax (602)266-8227, e-mail bds@bdsltd.com or mail.

Option 1: DOWNLOAD BEHAVIOR DATA SYSTEMS AGREEMENT

1. To download the agreement, you need to understand you are sending a Microsoft Word document from this website to your computer. By clicking on the Agreement link, you will then see a 'File Download' message box display on your screen like the one shown above:

| 2. The next step is to click on the 'Save' button. |

3. You will see another 'Save As' message box open up like the one in the picture above. Use your mouse arrow to open the 'Save In' pull down and select the 'My Documents' folder. Then, click on the 'Save' button in the lower right corner.
This will be the folder that the 'agreement' document will be stored in. You then can print the document from your Microsoft Word program, fill it out, sign the bottom and fax it to: (602) 266-8227. |

Option 2: PRINT DIRECTLY FROM THE WEBPAGE
1. Be sure your printer is on and has paper.
2. Click on the 'Print Agreement ' button below.
3. Another window will display with the Agreement Form inside. You will also see a Print Dialog Box display just after . Click on the 'OK' button within the dialog box to continue and print the form.
4. Close the form by clicking on the 'X' button in the upper right corner of the form, and you will return to this webpage.

TERMS AND CONDITIONS OF USE
1. It is specifically acknowledged that all Behavior Data Systems, Ltd. (BDS) products, including tests, diskettes and software programs are copyrighted and ALL RIGHTS ARE RESERVED.
2. BDS software programs contained on BDS diskettes are copyrighted and must not be copied or loaded on a hard drive or mainframe computer equipment without written permission from Behavior Data Systems.
3. BDS diskettes shall remain the property of BDS and shall be returned to Behavior Data Systems when used, damaged or recalled.
4. Damaged diskettes or malfunctioning diskettes shall be replaced within a 30 day warranty period and unused test applications shall be credited to the client account. Unused test credits are valid for one year. No other warranties are expressed or implied.
5. The test unit fee of $_________ U.S. currency per-test application applies throughout the duration of this agreement. Payment shall be made directly to Behavior Data Systems within 30 days of receipt of ordered materials, unless other arrangements have been made.
6. All BDS tests are to be used in conjunction with an interview of the respondent by experienced staff. And, no decision or diagnosis is to be made solely on the basis of Behavior Data Systems tests results.
7. This BDS agreement is non-transferable and cannot be transferred to any third party.
8. In no event will BDS, its owners or officers be liable for direct, indirect, special, incidental, punitive, exemplary or consequential damages arising out of the responsible parties signiture use or failure to use BDS products, beyond replacement of defective, damaged or recalled diskettes.
My signature certifies my understanding, agreement and compliance with the terms and conditions of this agreement. By signing this agreement, I acknowledge that I have read and understand the provisions contained herein, and that I agree to be bound thereby.
Behavior Data Systems diskettes are to be returned when used or after they have been on-site for one year. This is because BDS assessment instruments are updated annually.
